Duties Exercise professional use of variety of casework skills in developing comprehensive psychosocial assessments which examine the current situation as well as relevant bio- psychosocial and environmental factors of patients and their families; with priority of work done as inpatients and includes outpatients as well Performs case management functions including information and referral services and linkages to appropriate hospital resources as well as community based organizations. Collaboratively formulates appropriate and realistic discharge plans working with other members of the health care team and available community based organizations. Participates in continuing education programs to fulfill licensure requirements and to promote professional growth. Participates in CQI and TJC activities including policy implementation and revisions. Utilize supervision and consultation for professional growth and to provide more effective services to patients and their families. Performs other duties as assigned. Master's degree with practicum: Applicants who complete all of the requirements for a master's degree in social work that included a practicum are eligible for GS-9. Licensure: Must possess and maintain a copy of an unrestricted/unexpired license for Social Work In addition to the Basic Requirements, you must also meet the Minimum Qualifications stated below. MINIMUM QUALIFICATIONS: Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the next lower grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: SPECIALIZED EXPERIENCE: Experience as a Social Workers in a hospital setting or clinic setting. Providing counseling to clients and families, assistance to groups and community, making independent and professional decisions with recommendations for patient care, planning while helping patients to understand and modify patterns of behavioral to find resolution, exercises alertness and resourcefulness, initiative and mature judgement in program implementation. Time In Grade Federal employees in the competitive service are also subject to the Time-In-Grade Requirements: Merit Promotion (status) candidates must have completed one year of service at the next lower grade level. Time-In-Grade provisions do not apply under the Excepted Service Examining Plan (ESEP). You must meet all qualification requirements within 30 days of the closing date of the announcement.
