Need profiles by 7/15Candidate Profile: Curriculum vitae, resumes, licensures, and all relative documents for proposed candidates. -Registered nurse (Risk Management) services to the Quality Services Department, Northern Navajo Medical Center, Highway 491 North, Shiprock, New Mexico, 87420 -PERIOD OF PERFORMANCE: Date of Award through one year. -HOUSING: No government housing available. . The nearby towns are Farmington, NM , 29 miles East of Shiprock, NM and in Cortez, CO, 41 miles North of Shiprock, NM. There are hotels, restaurants, mall, etc. in the two towns. - -5.0.1 - - - -The contractor shall provide professional risk management services as follows: -A. - - - - - - - - - - - -Knowledge of professional nursing concepts, principles, and practices pertinent to performance improvement which affect patient care. -B. - - - - - - - - - - - -Develops, plans, and administers an internal risk management plan which includes coordination of all aspects of hospital loss prevention and liability control, risk identification, remedial action, documentation, education and related issues at the Northern Navajo Medical Center and its off-site clinics. -C. - - - - - - - - - - -Collects and analyzes data to monitor the performance of processes that involve risk or that may result in serious adverse events (e.g., preventive screening, diagnostic testing, medication use processes, and prenatal care). -D. - - - - - - - - - - -Uses proactive risk assessment tools that can include the use of failure mode and effects analysis, system analysis, and other tools to identify and mitigate areas of risk. -E. - - - - - - - - - - - -Analyzing data collected on adverse events, near misses, and potentially unsafe conditions; providing feedback to providers and staff; and using this data to facilitate systems improvements to reduce the probability of occurrence of future related events. Root cause analysis and systems analysis can be used to identify causes and contributing factors in the occurrence of such events. -F. - - - - - - - - - - - - -Ensures compliance with data collection and reporting requirements of governmental, regulatory, and accrediting agencies. -G. - - - - - - - - - -Facilitates and ensures the implementation of patient safety initiatives such as improved tracking systems for preventive screenings and diagnostic tests, medication safety systems, and falls prevention programs. -H. - - - - -Systematically monitors and evaluates the quality of patient care, continuous improvement of patient care and support of service unit strategic plan. -I. - - - - - - - - - - - - - - - - - - -Conducts health system risk/quality management surveys and analysis. Applies management principles, concepts, theories, practices, techniques, and methodologies where operational activities are characterized by features as interrelated functional assignments, new or modified work process, and control systems involving hospital and satellite clinics. -J. - - - - - - - - - - - - - - -Determines risk events from information and provides specific recommendations to the Executive Leadership Team for evaluation and action. Ensures investigation of all risks involving actual or potential injury to patients, visitors, and employees. This responsibility involves working with pertinent personnel to establish criteria and standards for highest achievable quality of health services. Assists the patient representative with resolution of complex patient issues providing legal resources as needed. -K. - - - - - - - - - - - -Communicates with the Chief Executive Officer (CEO), Chief Medical Officer, Chief Operating Officer, Chief Financial Officer, Chief Nurse Executive, and other key staff members as needed. Informs and provides input to all regulatory issues/findings/surveys to the executive team department managers to eliminate all levels of risks; assist clinical directors and department managers in designing safe, quality care processes. -L. - - - - - - - - - - - - - -Discusses and makes decisions on policy making involving the hospital self- - assessment programs concerning potential risk. Develops ana lyrical material to provide necessary data to assist leadership on organizational structure and management systems. Provides leadership and direction on approach, technique, and standard analytical procedures for problem solving to meet evaluation and mitigation of risk. -M. - - - - - - - - -Responds to crisis situations that have risk management implications to assist staff. Answers medical/legal inquires of physicians, nurses, and administrators regarding emergent patient care issues and loss control. N. - - - - - - - - - - -Adheres to guidelines for IHS, TJC, CMS, and other accreditation standards, policies, procedures, and regulations applicable to hospital al self - assessment risk management programs. Regularly updates the (Hospital Executive Committee (HEC) on current guidelines. -0 . Contractor shall have access to the internet to use the Electronic Health Record. -P. Contractor shall be subject to the provisions and shall be monitored by the Northern Navajo Medical Center (NNMC) Center's Quality Assessment and Performance Improvement (OAPI) Plan. -5.1 - - - - -Work Schedule: -5.1.1 - - - -The Supervisor of the Quality Department shall determine specific tour of duty the contractor will work. The contractor shall work 100% of the contracted hours. -5.1.2 - - - -Work Flexibility. As directed by the Supervisor, the Contractor shall coordinate with the Quality Department Supervisor to design the tour hours that support the risk management program. This is typically Monday - Friday, 8:00 a.m. - 4:30 p.m. but may change to support the risk management objectives for the hospital. -
