Need profiles at 8/19Registered nurse (Risk Management) services to the Quality Services Department, Northern Navajo Medical Center, Highway 491 North, Shiprock, New Mexico, 87420 -PERIOD OF PERFORMANCE: Date of Award through one year. -Candidate Profile: Curriculum vitae, resumes, licensures, and all relative documents for proposed candidates. -HOUSING: No government housing available. . The nearby towns are Farmington, NM , 29 miles East of Shiprock, NM and in Cortez, CO, 41 miles North of Shiprock, NM. There are hotels, restaurants, mall, etc. in the two towns. -The incumbent of this position establishes, directs, coordinates, and evaluates the Service Unit's Risk Management/Patient Advocate Program through the Patient Advocate. The purpose of this position is to oversee and coordinate all administrative management and support services for the Risk Management and Patient Grievances programs. The work includes development of plans and administers management plan which includes coordination of all aspects of hospital loss prevention and liability control, risk identification, remedial action, documentation, education and related issues. Monitors the effectiveness and performance of risk management and patient safety actions through analysis, investigation, reporting, and compliance to policy/procedure and regulatory agency requirement; Develops policies and procedures; application of standards and management principles; research and evaluation -of operations; Performance of duties involves establishing criteria, assessing program effectiveness, investigating or analyzing a variety of conditions, problems, or questions, which affect a wide range of activities, and stimulating responsive action toward achievement of quality care. Responsible for reporting to and collaborating and coordinating with legal attorneys, management, and governing bodies. -Develops, plans, and administers an internal risk management plan which includes coordination of all aspects of hospital loss prevention and liability control, risk identification, remedial action, documentation, education and related issues at the Northern Navajo Medical Center and its off-site clinics. -Ensuring the collection and analysis of data to monitor the performance of processes that involve risk or that may result in serious adverse events (e.g., preventive screening, diagnostic testing, medication use processes, and prenatal care). Proactive risk assessment can include the use of failure mode and effects analysis, system analysis, and other tools. -Analyzing data collected on adverse events, near misses, and potentially unsafe conditions; providing feedback to providers and staff; and using this data to facilitate systems improvements to reduce the probability of occurrence of future related events. Root cause analysis and systems analysis can be used to identify causes and contributing factors in the occurrence of such events. -Ensuring compliance with data collection and reporting requirements of governmental, regulatory, and accrediting agencies. -Facilitating and ensuring the implementation of patient safety initiatives such as improved tracking systems for preventive screenings and diagnostic tests, medication safety systems, and falls prevention programs. -Systematically monitors and evaluates the quality of patient care, continuous improvement of patient care and support of service unit strategic plan. -Conducts health system risk/quality management surveys and analysis. Applies management principles, concepts, theories, practices, techniques, and methodologies where operational activities are characterized by features as interrelated functional assignments, new or modified work process, and control systems involving hospital and satellite clinics. -Monitoring the effectiveness and performance of risk management and patient safety actions. Performance monitoring data may include: -Claims and claim trends Culture of safety surveys Event trending dataOngoing risk assessment informationPatient's and/or family's perceptions of how well the organization meet s their needs and expectations Quality performance dataResearch data -Determines risk events from collected information and provides specific recommendations to the Executive Leadership Team for evaluation and action. Ensures investigation of all risks involving actual or potential injury to patients, visitors, and employees. This responsibility involves working with pertinent personnel to establish criteria and standards for highest achievable quality of health services. Assists the patient representative with resolution of complex patient issues providing legal resources as needed. -Provides early analysis and investigation of actual as well as potential risks within and around the area served. Makes periodic inspections to achieve compliance and reports any issues to the CEO, Director of Quality Management and Hospital Executive Committee (HEC) for corrective action. -Communicates with the Chief Executive Officer (CEO), Chief Medical Officer, Chief Operating Officer, Chief Financial Officer, Chief Nurse Executive, and other key staff members as needed. Informs and provides input to all regulatory issues/findings /surveys to the executive team department managers to eliminate all levels of risks; assist clinical directors and department managers in designing safe, quality care processes. -Discusses and makes decisions on policy making involving the hospital self-assessment programs concerning potential risk. Develops analytical material to provide necessary data to assist leadership on organizational structure and management systems. Provides leadership and direction on approach, technique, and standard analytical procedures for problem solving to meet evaluation and mitigation of risk. -Responds to crisis situations that have risk management implications to assist staff. Answers medical/legal inquires of physicians, nurses, and administrators regarding emergent patient care issues and loss control. -Interacts with legal counsel, patients/families. -Works in close alliance with attorneys who present subpoenas. Provides requested records for review and works with the US attorneys who represent the facility regarding Tort Claims. Assists in preparation of staff involved in the Tort Claim process who may be called for deposition or trial. -Chairs the Risk Management Committee. Serves as member of the Quality Management Team, Corporate Compliance Committee, Safety Committee and coordinates regularly with Health Information Systems reviewing subpoenaed records for completeness and provides analysis to NAIHS for approval. Incumbent coordinates with the compliance officer concerning regulatory requirements and protection of patient rights, HIPAA and informed consent. -Conducts or participates with other department heads in the orientation of employees concerning the Risk Management program utilized by the hospital. Develops formal program for ongoing education for hospital staff. -Adheres to guidelines for IHS, TJC, CMS, and other accreditation standards, policies, procedures, and regulations applicable to hospital self-assessment risk management programs. Regularly updates the HEC on current guidelines. -
