Job Description Job Description We are seeking a dedicated and compassionate Case Manager to join our client in San Jose. The Case Manager will be responsible for providing direct support to clients, promoting prevention and education. Job overview: Hiring method: Contract / Temporary ?Location: 100% onsite in San Jose Hours: Tuesday - Saturday, 9am-530pm Key Responsibilities: Manage a caseload of 20-25 clients, assisting clients through application process for Interim Housing team Perform client intake and assessment determine best plan of service for each client Promote prevention, education and healthy activities in collaboration with other teams Perform outreach activities and educational services, crisis intervention Documentation of case notes in HMIS system Qualifications: Minimum of 1 year of experience in case management or a related field in social services Strong understanding of issues related to homelessness, poverty, and trauma-informed care Excellent communication, organizational, and problem-solving skills.
