Job Description FM opportunity with a well establish and growing health program. The position is a permanent (full-time) opportunity located in the community of San Jose, CA. Responsibilities include providing primary care, with emphasis on healing maintenance & preventive care. Site practices a Patient Centered Health Home and all employees are an integral part of this model of care delivery. No call, no weekends, no inpatient, no after hours required! This multi-site health program provides high quality, culturally competent outpatient care to an underserved patient population, so a rewarding chance to impact the health of a community. Comprehensive services including medical, dental, pharmacy, behavioral health, lab and other community health and outreach programs. Salaried, full time position ($210K-$260K) includes benefits: paid holidays, paid vacation/sick, license/dues, CME days/$, health insurance, covered malpractice, retirement matching with loan repayment available. Candidates must have/obtain active CA license, current DEA and BE/BC in FM. Must be able to treat children and adults. Work Visa Sponsorship is available.
